Advice Only Initial Consultation

All patients seeking an appointment for 'advice only' to review symptoms and/or concerns may do so without submitting all mandatory blood work, physical exams, medical records, and history (although recommended). An assessment and treatment plan will be RECOMMENDED, but no medications or prescriptions will be released if required documents are not provided to us beforehand. If this consultation type is requested, a consent form will be sent for you to sign to confirm your understanding.
