Renewal Consultation- Saya

In the instance that a patient does not stay up to date with their treatment protocol (blood work, follow-up consultations, medication refills), we will consider them a 'renewal' patient after 6 months of no activity in their chart. This will entail having an initial panel of blood work completed, submitting a recent physical exam, and re-submitting any forms that need updating. Once all requirements are received, the patient is scheduled for a Renewal consultation with Dr. Saya to re-commence their protocol.
